School-Based Telehealth for Students


Beginning in the 2026-2027 school year, Concho Medical Clinic will offer medical care at school for Eden CISD and Paint Rock ISD students through telehealth.
This program gives students a convenient and timely way to get medical needs addressed.

Student Telehealth Information & Consent Forms


Before a student can participate in school-based telehealth services, a parent or guardian must complete a consent form.
Please select the correct form below and download it to fill out.
Completed forms can be delivered to your school's front desk, or they can be dropped at Concho Medical Clinic located at 551 Eaker Street, Eden, Texas 76837.

Before receiving any telehealth service at school, the student's parent or guardian will be notified and have the opportunity to deny the request.

What is School-Based Telehealth?


School-based telehealth allows a student to connect with a Concho Medical Clinic clinician by secure video while at school for minor medical concerns, including:

  • Headaches

  • Allergies

  • Rashes

  • Management for chronic conditions like asthma or diabetes

  • Sore throats

  • Infections


How the Process Works



  1. Assessment: The student visits the school nurse, who determines if a virtual clinician visit is necessary.

  2. Parental Consent: The nurse contacts the parent or guardian for consent.

  3. Virtual Consultation: A nurse practitioner or doctor performs a video visit.

  4. Treatment: The clinician sends any needed prescriptions directly to the pharmacy and updates the child's primary care provider.
