Care That Can Come to You

Concho Medical Clinic now offers scheduled home visits for patients within the City of Eden who may have difficulty coming into the clinic.

Home visits are available for both new and established patients when clinically appropriate and based on clinician availability. This service may be helpful for patients who are homebound, older adults, people with disabilities, patients managing chronic conditions, people facing transportation barriers, and patients who may otherwise have difficulty with in-person clinic visits, including some patients with sensory sensitivities.

Home visits are provided by Concho Medical Clinic’s full-time clinicians, Roy Guevara, MSN, FNP-C, and Harper Price, DNP, APRN, FNP-C.

When Home Visits May Help

A home visit may be appropriate for patients who:

Have difficulty getting to the clinic
Are homebound
Have mobility challenges
Have a disability or chronic condition
Face transportation barriers
Need follow-up care after an emergency room visit
Have difficulty with in-person clinic visits

ER Follow-Up Care

One of our clinic clinicians may reach out after your emergency room visit to discuss recommended next steps or ongoing care needs. A home visit may be available when clinically appropriate.

Billing and Insurance

Home visits are billed the same as clinic visits. Insurance is accepted, including Medicare and Medicaid.

Important Information

Home visits are intended for non-emergency medical needs that can typically be addressed in a clinic setting. They are not available for emergencies or medical needs that require immediate emergency care.
